Support groups are registered with the City Boy Movement through this application, which collects the documents the National Support Group Directorate requires for recognition. You may complete it in stages; your progress is saved as you go.

- 4Signed Minutes of meetings
- 5Executive Officers and their positions
- 6Valid ID of the Principal Executive officers
- 7Current Membership register
- 8Evidence of official address or meeting venue
- 9Activity plan up to the general election
- 10Photo Evidence of your support group activities
- 11Declaration of accuracy and completeness
- 12Affiliation and conflict of Interest declaration
